Application Procedure to Conduct Human Research

To apply for approval of research involving human subjects, the principal investigator must submit an application to the HSRB (Human Subjects Review Board)--through the chairperson of the HSRB.

Printed forms are available in the office of the Academic Vice President and the office of Institutional Research. The forms may also be downloaded from this web page.

To determine which forms are appropriate for your research please complete the following questionaire by clicking the appropriate buttons.

You responded:

  1. The research involves human subjects.

  2. You are a student under supervision of an advisor in a research class or a student in a class preparing protocols for limited data collection or pilot studies.

If the above information is correct, submit the following forms:
1. "Application for Approval of Research Involving Human Subjects"
2. "Brief Research Protocol"
3. "Informed Consent" Documentation (When indicated)
