For ethical and legal reasons, documentation of informed consent must receive particularly careful attention. It must include any documents, questionaires, or instruments used to conduct the research, such as:

• A Cover-Letter of Explanation.
• Questionnaires.
• A Written Copy of Verbal Instructions.
• Letters of Permission
